Job Summary:
We are seeking a dedicated and dynamic Occupational Therapist to join our Community Reablement (Rehabilitation) Team in an inpatient setting. The successful candidate will work with adults to provide high-quality, patient-centred care aimed at enhancing independence and quality of life. This role involves assessing, planning, implementing, and evaluating interventions to support patients in their rehabilitation and reablement journey.
Key Responsibilities:
- Assessment and Diagnosis: Conduct comprehensive assessments of patients’ physical, emotional, and social needs to determine their rehabilitation and reablement goals.
- Therapeutic Interventions: Develop, implement, and evaluate individualised treatment plans based on patient needs and evidence-based practices.
- Patient-Centred Care: Work closely with patients to set realistic and achievable goals, facilitating their active participation in the rehabilitation process.
- Multidisciplinary Collaboration: Collaborate with a multidisciplinary team, including physiotherapists, nurses, social workers, and other healthcare professionals, to deliver integrated care.
- Reablement Focus: Promote independence and well-being by focusing on enabling patients to perform daily activities and routines.
- Education and Support: Provide education, support, and advice to patients and their families to ensure a smooth transition from inpatient care to home or community settings.
- Documentation: Maintain accurate and detailed patient records, documenting assessments, treatment plans, progress notes, and outcomes.
- Caseload Management: Manage a defined caseload independently, maintaining high standards of care and ensuring effective prioritisation and time management.
- Professional Development: Engage in continuous professional development, staying current with advancements in occupational therapy and rehabilitation practices.
Essential Qualifications and Experience:
- Education: Bachelor’s or Master’s degree in Occupational Therapy or a related field.
- Registration: Current registration with the Health and Care Professions Council (HCPC).
- Experience: Relevant experience working within rehabilitation or reablement settings, preferably within an inpatient or community context.
- Competencies: Strong assessment, diagnostic, and therapeutic skills with the ability to develop and implement effective reablement plans.
- Skills: Excellent communication, interpersonal, and organizational skills. Ability to work effectively both independently and as part of a multidisciplinary team.
- Flexibility: Ability to adapt to changing environments and work effectively under pressure.
Desirable Qualifications and Experience:
- Band 6 Candidates: Additional experience and demonstrated competence in leadership roles, complex case management, and supervision of junior staff.
- Specialised Training: Further training or certifications in specific areas of rehabilitation, reablement, or occupational therapy practices.
- Professional Development: Commitment to ongoing professional development and contribution to the development of the reablement team.
